Information for Matching Gifts

To double – or even triple - the impact of your donation, make sure to apply for your company's matching gift!

The steps are simple:

1. Check with your company to see if they will match your contribution to the Alzheimer's Association. Your Human Resources department will have this information available. Note: many employers also match gifts made by spouses or retirees.

2. Once you have determined your company matches gifts made to the Alzheimer's Association, request a matching gift form, complete, and mail to:

   Watertown, MA 02472
   Attention: Matching Gifts

3. The Alzheimer's Association will confirm receipt of your donation and apply for the  matching gift from your company.

For your information, the Alzheimer's Association MA/NH Chapter's Tax Identification Number (also known as Employee Identification Number or EIN) is 04-2731194.
